One I didn't see in there is Garden State Parkway at NJ 3 - NB over, SB
under. (The cool thing about this is, both 3 and old 3 - Allwood Road -
pass the same way. Also, there's an orphaned state route that begins
off Allwood Road that was once part of the 3/S-3 (spur) routes.)

 I-39/90/94 north of Madison, WI, Columbia County.
Sanderson Road goes over NB and under SB
The freeway lanes are on opposite sides of a little valley.

I too, think this type of grade separation is pretty sweet.

The Taconic State Parkway at the Underhill Ave. exit near Yorktown Heights
is like that. The original, northbound lanes of the TSP have an overpass
over Underhill Ave., while Underhill Ave. passes over the southbound lanes
of the TSP.

IIRC I-71 at Ky 329, near Crestwood Ky Qualifies
